I live in Milan, I take this route more often than I care to admit, and I will tell you plainly: for the roughly 50 km haul from the airport into the middle of the city, the train is the answer nine times out of ten. Cadorna — properly Milano Cadorna FN, the terminus wedged right against the Castello Sforzesco and plugged into both the M1 and M2 metro lines — is the western end of the Malpensa Express line. That is not a coincidence. The train was built to land you here. Everything else is a compromise you make for a reason, and I will walk you through those reasons honestly.

The Malpensa Express, and why I rate it above everything

The Malpensa Express is run by Trenord, and it does the airport-to-Cadorna leg in about 38 minutes. Fifteen euro for an adult. If you are travelling with kids aged 4 to 13, they go for roughly €7.50, which is worth knowing before someone at a machine sells you full-price tickets you did not need. Departures come round every half hour or so, across a generous window from about 05:00 until 23:30, so unless your flight has actually crawled in at some ungodly hour, there will be a train. You catch it at the underground rail station beneath Terminal 1 — follow the signs down, it is well marked. If you land at Terminal 2, that terminal has its own station stop, so you are not forced to shuttle across first.

Here is the part that matters and that no taxi driver will volunteer: the train does not sit in traffic. The tangenziali around Milan can turn a 50-minute drive into an hour and a half without warning — a match at San Siro, roadworks, a wet Friday evening, take your pick. The train ignores all of it. Thirty-eight minutes is thirty-eight minutes whether the roads are clear or gridlocked. When I have a meeting to make, that predictability is the whole ballgame. One word of warning, because I have watched tourists learn it the hard way: buy and validate your ticket before you board, from the machines or the app. Boarding without a valid ticket earns you a fine, and "I did not understand" is not a defence the conductor accepts.

The white taxi and the flat fare that is actually fair

If you are not a train person — too much luggage, a body that stopped cooperating three time zones back, or you simply cannot face a metro connection at the other end — the licensed white taxis are a solid fallback. You will find the official rank right outside Arrivals at Terminal 1, and at Terminal 2 as well. Milan runs a regulated flat fare from Malpensa into central Milan of roughly €104 to €110. No meter ticking, no nasty night surcharge sprung on you at midnight. That said, do the sensible thing and confirm the fixed tariff with the driver before you pull away. The fare is set, but a quick word up front spares you any argument later. Expect about 50 minutes door to door in decent conditions, and remember that "decent conditions" is exactly the variable the train removes.

On cost: solo, that hundred-euro-plus fare is frankly absurd next to a €15 train ticket. But split three or four ways among a family or a group with a pile of suitcases, the taxi starts to look reasonable, and it drops you at your actual front door rather than at Cadorna's forecourt. That is the short trade.

Private transfers — for the arrival that needs babysitting

The third option is a pre-booked private transfer, through outfits like Welcome Pickups or various local firms. You arrange it in advance, a real driver rather than an app match is there to shoulder your bags and already knows the way in, and you are quoted a fixed price up front, roughly €90 to €130 depending on the operator and the car. The journey time mirrors the taxi at around 50 minutes. Where this justifies the cost is in the fiddly cases: a late-night arrival when you would rather not gamble on cab availability, a mountain of luggage, or a flight you suspect will be delayed — the good operators track your flight and build in waiting time, so a slow passport queue does not leave your driver gone. It costs more than flagging a taxi, but you are paying for the certainty.

My verdict, without the hedging

Take the Malpensa Express to Cadorna. Fifteen euro, thirty-eight minutes, traffic-proof, dropping you at a metro-connected terminus right in central Milan. If you are a group with heavy bags, the flat-fare taxi at roughly €104–110 is defensible once you divide it up. If your arrival is late, luggage-laden or delay-prone, book a private transfer for peace of mind. But for the solo traveller, the couple, anyone who can wheel a case to a platform — the train wins, and it is not close.
