Fifty kilometres between the runway and the rose window

You did not fly to Milan for the periphery. You came for the marble filigree of the Duomo, for the glass dome over the Galleria Vittorio Emanuele II, for the disciplined windows of the Quadrilatero della Moda where Montenapoleone and Sant'Andrea keep their quiet theatre. And yet the city keeps you waiting: Piazza del Duomo sits roughly 50 km southeast of Malpensa, an hour or so of Lombard plain between the terminal doors and the cathedral steps. How you cross that distance sets the register for everything after — whether you arrive composed, or arrive frazzled with a wheelie case wedged in a turnstile. Below, the four ways in, each judged the way you would judge a cut of cloth: by fit, by finish, by what it costs.

The Malpensa Express, and the single red stop that finishes it

For the traveller who values precision, the train is the obvious protagonist. The Malpensa Express (Trenord) leaves from the underground platform beneath Terminal 1 (Terminal 2 has its own stop) running roughly every half hour from around 05:00 to 23:30, immune to the caprices of motorway traffic. It will not, however, deposit you at the cathedral; no rail line does. The elegant move is to ride to Milano Cadorna, then step onto metro line M1, the red one, for the couple of stops that end at Duomo. Allow about 55 minutes all told and roughly €17: the ~€15 Malpensa Express fare plus a ~€2.20 ATM urban metro ticket. Buy both from the machines or the app before you board — Italian conductors are unsentimental about an unvalidated ticket, and a fine is a graceless way to begin. This is the choice for the solo arrival and the light packer, and it stays cheap in a way the meter never will: two of you ride in for around €34, where a cab is a flat three figures regardless.

The shuttle coach, for the budget that still wants the view

If the euros matter more than the minutes, the Malpensa Shuttle earns its place. Autostradale and Terravision coaches board from the bays just outside Arrivals and run every 20 to 30 minutes to Milano Centrale — that cathedral of iron and travertine in its own right. From Centrale you drop underground onto metro line M3 and glide to Duomo. Budget about €12 in total (~€10 for the coach, ~€2.20 for the metro) and around 65 minutes, a little slower once you factor the transfer at Centrale. It suits the unhurried aesthete counting pennies for the boutiques rather than the transfer, though heavy traffic can stretch the road leg in a way the train shrugs off.

The white taxi and its civilised flat fare

Milan does one thing for the airport arrival that few cities manage with such restraint: the official white taxis charge a regulated flat fare, roughly €104–110, from the rank outside Arrivals straight into the central zone, and that zone comfortably includes the Duomo and the fashion quarter around it. No meter theatrics, no night surcharge, no negotiation beyond confirming the fixed tariff with the driver before you pull away. Expect about 55 minutes door to door, traffic permitting. For one person it is an indulgence; for three or four sharing, split across the fare, it starts to look almost sensible, and the appeal sharpens the moment your luggage does. The historic centre is threaded with limited-traffic zones and pedestrianised stone — a car that sets you down as close as regulation allows spares you dragging a hard case across the cobbles of the Quadrilatero.

The private transfer, and the case for arriving unbothered

The most composed entrance of all is pre-booked. A private door-to-door transfer through Welcome Pickups or a local firm has you walk straight from the Arrivals hall to a waiting car and carries you, in roughly 55 minutes, to the door of your central hotel for a fixed quoted price around €90–130, flight tracking and a measure of waiting time typically built in. Against a stack of separate train-plus-metro tickets it reads as extravagant for one; split three or four ways it can rival (occasionally undercut) the flat-fare cab while sparing you the ticket machines and the transfer entirely. It is the answer for the late arrival, too, when a delayed flight lands you past the last comfortable train and you would rather not decipher a night-time transfer with your guard down.

What I would do

Travelling light and alone, take the Malpensa Express to Cadorna and let one stop on the red line hand you to the Duomo — fast, honest, about €17. Counting every euro, ride the coach to Centrale and drop onto M3. Landed late, laden, or moving as a group of three or four, book the private transfer or take the flat-fare taxi and let the fare become bearable divided; either way you step out at your door with your case intact and your composure with it, ready for the cathedral, the Galleria, and those unforgiving vitrine on Via Montenapoleone.
